Pictured above in addition to Community Health Director, Emily Payne (left in white) are nurses, navigators, and community health workers at our Transition Clinic.  These ladies are "boots on the ground" in the battle to make our community healthier.  United Regional's COMMUNITY CARE FUND helps them serve patients daily.

The COMMUNITY CARE FUND primarily helps discharged hospital patients who have ongoing health needs yet lack the means to meet those needs due to challenging socio-economic circumstances. These patients often receive support through the Transition Clinic, where navigators, advocates, and providers stop at nothing to improve their health outcomes and connect them with long-term healthcare solutions. In 2024, the Transition Clinic served over 3,500 patients, many of whom benefited from the COMMUNITY CARE FUND.

The COMMUNITY CARE FUND provides essentials such as medications, diabetic supplies, transportation to medical appointments, food, medical equipment, specialty care access, and more. This assistance is particularly crucial for those aged between 21-64, who may not have access to medical care elsewhere in the community.

Hospital funds cannot be allocated for these types of supplies and services for patients under Medicare, Medicaid, and Tricare due to federal restrictions. Contributions to this Fund are therefore critical to ensuring that all patients, receive the help they need.
